YMCA Health Fitness and Aquatics Member Survey Incentive Prize Draw Contest General Rules
HOW TO ENTER
NO PURCHASE NECESSARY. Entrants must provide valid contact information on the survey form and correctly answer a mathematical skill testing question. Entrants will receive one contest entry for completing the survey. Additional survey completions will not result in additional contest entries.
CONTEST PERIOD
The contest is open from October 7, 2024 to November 1, 2024.
SELECTION OF WINNER(S)
Winner(s) will be selected by your YMCA, on or before November 15, 2024. Winner(s) will be notified by telephone or electronic mail no later than November 30, 2024. The odds of winning are dependent on the number of people entering the contest.
PRIZE(S)
Please see the contest entry form in the survey for a full description of the prize(s) being awarded in your YMCA and their approximate cash value. Prize(s) must be accepted as awarded, no substitutions or exchanges are allowed. Prize(s) consisting of YMCA services (memberships, camp registrations, etc.) have no cash value and must be accepted as awarded or forfeited.
GENERAL
The YMCA will use the entrant’s personal information in accordance with the YMCA’s current privacy policy, which is available for viewing on the YMCA’s web site, or upon request at the YMCA.
By participating in the contest, each Entrant releases and holds harmless the YMCA, and each of their affiliates, representatives, and their respective officers, directors, employees, representatives, and agents, (the “Releasees”) from any and all liability for any injuries, loss or damage of any kind to the entrant or any other person, including personal injury, death, or property damage, resulting in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of any Prize, participation in the contest, any breach of the Rules, or in any Prize-related activity. Each Entrant agrees to fully indemnify the Releasees from any and all claims by third parties relating to the contest, without limitation.
